Lancashire will stay in Division One for another season and relegate Middlesex if they win the match between the two sides at Old Trafford.
Resuming day three on 259-6, the Red Rose needed 41 more runs for a third batting point and to ensure Middlesex did not claim a third bowling point.
Tim Murtagh took two early wickets to leave the hosts struggling on 271-8.
But Lancashire captain Glen Chapple and Tom Bailey saw them to 302-8 in the 110th over and immediately declared.
Needing seven runs from six balls, 40-year-old Chapple (45 not out) launched a huge six over mid-wicket off the first delivery to keep them in contention before collecting three from the next.
